Puppy Sitter Form
This form provides information when a puppy is staying at the home of another raiser/sitter. Examples: home switch, raiser goes on vacation, raiser goes on a business trip, etc.
The raiser downloads the form and saves it on their PC. The raiser completes Page 1 and emails it as an attachment to the raiser/sitter caring for the visiting puppy. It is important for the raiser/sitter to receive the form prior to the puppy arriving at their home so any questions may be addressed.
When the puppy returns home, the raiser/sitter completes Page 2 and emails it back to the raiser and to the Area Coordinator. The Area Coordinator will include the completed form in the puppy’s file so the puppy is credited with the home switch. Review any issues or problems that occurred while the puppy was away with your Area Coordinator.
